Job Description:

The position coordinates and provides clinical expertise and educational assistance of Medline wound and skin care products to hospitals, home health care facilities, nursing homes, as well as other areas of healthcare where wound and skin care are performed. Works successfully with Medline general line and health care sales force and wound care product specialists (WCPS). Assists the WCPS in their territories to increase sales in wound and skin care products by performing the duties described below.

  • Evaluates policies and procedures related to patient care in specialty area.
  • May visit patients and discuss specific problems with assigned clinicians and/or physicians. Does not provide direct patient care.
  • Participates with development, revision, and implementation of patient education programs, materials, and resources in specialty area.
  • Provides formal educational programs to meet staff needs on facility level as well as multi-customer basis in larger venues, such as hotels for groups up to 300 attendees.
  • Provides, coordinates, and presents education for sales of wound and skin care products on a local customer level to a national/corporate level.
  • Maintains current standard of practice in specialty area and seeks additional knowledge on unusual problems and conditions, new treatments, etc.
  • Demonstrates quality nursing care, problem solving skills, effective use of resources, collaboration with other health professionals, professional standards and peers within the Medline wound care product specialists team.
  • Represents the Wound Care Sales Division by making presentations and participating in forums, committees, and professional associations.

Job Requirements:

  • Bachelor's degree (BSN) in nursing required; Graduate of an accredited Wound, Ostomy, Continence Program and have achieved board certification (ie. CWOCN as a full scope WOC Nurse).
  • Minimum 5 years direct clinical experience as a CWOCN in a hospital, nursing home, wound care center or home health care setting or minimum of 5 years experience working for another manufacturer of wound and skin care products as a clinician.
  • Must have a high level background of wound and skin care products by category with the ability to learn the specific features of all Medline specific wound and skin care products.
  • Must be able to work independently with little to no direct supervision on a day to day basis.
  • Recent “hands on” wound and skin care experience highly desirable; some clinical administration experience a plus.
  • Strong oral and verbal communication skills required. Must have strong presentation skills.
  • Good working knowledge of Microsoft Office (Word and Powerpoint)
  • Must have a current nursing license, current board certification in WOCN
  • Must be able to travel 80% nationwide.

Overall Qualifications:

Qualifications:

  1. WOCN or ARNP preferred with current licensure
  2. A minimum of three (3) years of progressive experience in the acute care setting. Experience inclusive of education principles with wound and ostomy patients preferred.
  3. The ability to work and communicate effectively with professionals in diverse disciplines. Demonstrate competency in written and verbal communication skills across the continuum.

Specific Functions:

  1. Establish a system for understanding and communicating effectively with physicians involved in wound and ostomy care.
  2. Establish means of actively and continually exploring with physicians and patients their specific goals, objectives, problems, issues and needs.
  3. Be available as a resource to program staff, nursing, and ancillary department staff. Cross-trained in all aspects
  4. Lead and evaluate quarterly house-wide wound Prevalence study and report results to Director, NDNQI, and the hospital partner.
  5. Participates in the marketing and representation of Healthways to physicians, local, and national organizations and community groups.
  6. Maintain a positive relationship with the staff, patients, physicians, and hospital personnel.
  7. Participate in the ongoing evaluation of the nutrition portion of the program in order to maintain a state-of-the-art approach to wound/ostomy care and self-management training.
  8. Actively participate in and integrate current hospital structure, policy and procedures, and forms, as related to wound and ostomy care.
  9. Establish a system for coordinating wound and ostomy patient care throughout the entire episode of illness, spanning each geographical area of the hospital in which care is provided.
  10. Ensure all components of the program meet the Quality Assurance Standards as established by Healthways and the best evidence based care recognized by national organization oversight committees.
  11. Explores strategies to reduce the length of stay and resource consumption with wound and ostomy patients, facilitates implementation of the strategies house-wide and monitor house-wide results.
  12. Adapts daily or weekly schedule to meet the needs of the hospital partner.
  13. Reviews pertinent literature regarding wound and ostomy care and complications. Effectively communicate findings to team members and hospital staff.

The PeaceHealth Home Health Department, located in Eugene, is currently looking for a full time Patient Care Coordinator to be scheduled Monday through Friday, 8:00 am to 5:00 pm. Occasional weekend coverage may be needed for agency operations.

Job Purpose and Responsibilities:

In collaboration with Patient Care Manager, the Patient Care Coordinator facilitates the uninterrupted continuing medical care of the patient by efficiently and appropriately coordinating wound care services and resources necessary for the transition from inpatient facilities to home care. Coordinates the care of patients served by home care, utilizing the professional health care process of assessment, problem identification/planning, action/intervention and clinical goal/outcome evaluation.

Education:

Graduate of an accredited State Board of Nursing approved nursing program. BSN degree preferred.

Licensure/Certification:

Current Registered Nurse licensure in the State of Oregon required. Valid Oregon Drivers License and auto insurance is required. WOCN Certification required.

Experience:

A minimum of five years clinical experience and two years home health/hospice/home infusion experience required. Commensurate with educational preparation, demonstrates knowledge and ability to apply age specific principles of growth and development and like stages to meet each patient's needs for home health care. The ability to work in a fast pace, high volume environment is required. At least one year of experience as wound care specialist. Job competency will be demonstrated and documented per the attached tool within three months of hire.

The Enterostomal Therapist is accountable for direct and indirect quality care for patients of all ages, according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. The ET will function as an educator and consultant to the Home Care staff, patients, and in the community, as needed. The ET will coordinate patient care activities with other members of the health care team in a collegial relationship. The ET may need to travel outside the local area for educational opportunities.

Requirements include a current Oregon RN license, Enterostomal Therapist certification, BLS. Completion of an accredited course in Wound, Ostomy and Continence is required, but may complete after hire. Minimum 2 years nursing experience. Demonstrated knowledge of Enterostomal Therapy principles and procedures. Position requires limited travel inside and outside the Portland area.

Preferred: One-year home care experience (either prior to or upon certification as ET).

Using a collaborative approach, the Clinical Nurse Manager is a Registered Nurse responsible for the operation of the clinical activities of the Wound Care Clinic and Hyperbaric Unit (HBO). Under the supervision of the Director and in collaboration with the Medical Director, this position holds responsibilities for coordination of the clinical services of the clinic and HBO unit to provide consistent quality care for patients with wounds in the outpatient setting. He/She collaborates closely with physicians, outpatient and inpatient health care professionals and Wound Care Clinic Staff. Responsibilities include utilization of skills in organization, leadership, planning, implementation, evaluating and providing care of adult and geriatric clients. Is a role model for all in the patient centered care delivery framework. Complies with hospital, Wound Care Clinic and nursing philosophies/standards through the ability to perform job functions including but not limited to the common examples listed with each performance standard. Performs other duties as assigned. Follows PPH rules, policies, procedures, applicable laws, and standards. Carries out the mission, vision, values, and quality commitment of PPH. Minimum education BSN, or currently in progress of obtaining degree; MSN preferred. Minimum experience of five years related clinical experience; preferred experience of three years progressive management experience, prior financial management, and previous experience with wounds. Current CA RN and BLS required. Must take and pass wound certification when eligible.
