Guidance on OASIS

Effective January 1, 2017, Home Health Agency Providers and Vendors will begin using the OASIS-C2 form in submitting assessments. The Centers for Medicare & Medicaid Services guidance manual for OASIS-C2 is available here.

You can find the Society's OASIS resources here, including the 2016 WOCN Society's Guidance on OASIS-C2 Integumentary Items: Best Practice for Clinicians.

This guideline was developed by WOC nurses, who work in the home health setting, to facilitate the classification of wounds by home health clinicians.
